Office Policies

New Patient Registration

If you are a new patient, please download and complete the new patient paperwork. Please bring your completed paperwork to our office the day of your appointment or prior to your exam fax paperwork to 770-740-9338. We ask that you arrive 15 - 20 minutes prior to your appointment to process your paperwork and insurance information. Registration requires picture identification/driver's license and your current insurance card(s).

We ask that you bring in all medication that you take including prescribed medication and OTC medication including vitamins and supplements.

Please note that if you arrive more than 15 minutes late to your appointment, you may be asked to reschedule your appointment.

Billing

We participate in many insurance plans and we will file any insurance for you. Please note that the specific details of your benefits are between you and your insurance plan. You are encouraged to contact your insurance company prior to your visit for details regarding your insurance benefits. You are responsible for any deductible, co-payments and non-covered services and these are payable at the time of your visit. If your insurance is one in which we do not participate, you will be asked to pay for your charges in full.

Cancellation

Please call our office 24 hours in advance if you are unable to make your appointment or would like to reschedule.

Medication Refill Policy

Please be aware that our office requires a 24 hour notice to refill medications. Although we make every attempt to complete medication refills the day of your request, we cannot guarantee this. The number of refills given to you by your physician is usually sufficient to last until your next appointment and refills may not be authorized without proper follow-up.

You can expedite this process:

  • Let our medical assistants know during your visit of all upcoming refills needed.
  • Call your pharmacy for all refills so that the pharmacy can contact us directly. This insures that your refill will be accurate and efficiently handled.
  • Fill in your pharmacy number on your patient paperwork in case we need to contact your pharmacy.

Medical Records

In order to request a copy of your medical records, please fill out the form below and bring to the practice or fax it to us at 770-740-9338 and we will submit it to the copy service. Please allow two weeks for this to be completed.

Preparation for Physical Exam
